The length includes the external threads, so the size you select will be for a bar without counting the balls. This is the usual way to measure jewelry bars.
This straight barbell is made of surgical steel, so it's safe for your piercings.
Note that the bars thicker than 14g (1.6 mm) have a thread gauge of 14g (1.6 mm), so it's easy to find replacement balls for them.
