It's a straight ear gauge plug. It's shaped like a .357 Magnum cartridge that shoots .357-caliber bullets (a 9 mm bullet diameter).
Unfortunately, plugs and tunnels go by even numbers, so this plug is not the same size as the cartridge. However, it's available in 6 different sizes, from 2g to 5/8" (6 to 16 mm).
This item is made of surgical steel and comes with a gold plating that makes the plug look like a cartridge case. It comes with two o-rings that will secure it n your earlobe gauge.
