The labret you see here has a heart in the front, which has a thread on the back. The thread is screwed into the bar. That's the advantage of internally threaded bars. With external threads, the thread is screwed down over the bar, with the disadvantage that it can cause small scratches inside your piercing -- That won't happen with this labret.
The heart in the front is equipped with small lines in different colors. The colors can lead one's thoughts to LGBT groups, as it's precisely these colors that characterize the groups, in the form of the rainbow flag, for example.

It's a bit harder to screw, but the size is perfect, and it really looks good in the helix.
